Community Places provides residential accommodation and intensive support which positively encourages development. We strive to empower individuals to reach their optimum potential in life. Our aim is to be a centre of excellence, constantly seeking improvement in services, whilst always treating service users with respect and dignity.
We use positive strategies for managing people, for example, relationship management, person centred planning and involvement in choice of assigned workers. By achieving the right balance between service user empowerment and risk management with safety and support, we are able to provide high quality, person centred and cost effective care.
People who live at Community Places have access to a full range of support services, including Advocacy and Psychiatry.

Community Places provides a care package to service users who present with some challenging behaviours and complex needs. The majority of our clients have mild/moderate learning disabilities and these complex needs challenge mainstream services. Our staff are fully trained to support people with long and short term care needs with our ultimate goal to promote a more independent lifestyle.

Our mission at Community Places is to provide a robust holistic approach to care provision that supports individuals with developing and improving their life skills in preparation for more independent living.
We use a comprehensive care planning and risk assessment process to support and encourage individuals to meet their short and long term goals.
Personalisation is at the centre of the ethos of Community Places. Everyone we support has their own unique ‘person centred plan’. Working together with the individual, each plan is designed to support their own personal choices and aspirations, helping them believe in and realise their own potential. We have positive outcomes for everyone regardless of their age or presumed ability.
Our approach helps to empower individuals with reaching their optimum potentials in life. |
